Microsoft® Word Forms
If you do not have access to online survey software, you might want to consider using the "forms" tool in Microsoft Word. According to Microsoft, a form is: "A document that contains fill-in blanks, or form fields, in which you enter information. For example, you can create an online registration form in Microsoft Word that uses drop-down lists from which users can select entries." The forms tool will allow you to create surveys that you can "lock" and send out electronicially via email to your target audience. |
