Surveys & Survey Templates

According to the implementation manual, grantees could use surveys to collect their performance measure data.

The EMSC Program has created a set of surveys for each of the performance measures listed below:

  • Measure 71 (formerly 66a) – On-line Medical Direction for Pediatric Patients
  • Measure 72 (formerly 66a) – Off-line Medical Direction for Pediatric Patients
  • Measure 73 (formerly 66b) – Essential Pediatric Equipment for EMS Agencies
  • Measure 76 (formerly 66d) – Inter-facility Transfer Guidelines with EMSC Components for Critically Ill and Injured Pediatric Patients
  • Measure 77 (formerly 66e) – Inter-facility Transfer Agreements for Critically Ill and Injured Pediatric Patients

The survey instrument has been tested and piloted. All states are required to use the EMSC Program survey templates UNLESS they are approved by HRSA to collect data from a different source.

 

Acquiring the Approved EMSC Survey Templates

EMSC grantees, if they choose to collect data via a survey, must use the EMSC Program approved survey instrument. Grantees should contact their NEDARC representative who will provide them with the survey template and will work with them one-on-one to customzie the survey for their state/territory.
